Upgradation of Lymph Supermicrosurgical Skills through Lymphatic Venous Anastomosis

چکیده

Supermicrosurgery, a technique involving the dissection and anastomosis of small vessels, has revolutionized field lymphedema treatment. However, in lymphatic venous anastomosis, an arduous is required to manipulate 0.1-0.8-mm channels, whereas conventional microsurgery deals with 0.8-2.0-mm vessels. Although several practice models lymph supermicrosurgery are available, upgradation supermicrosurgical skills human not been reported. In this study, we suggest efficacy steps for surgeons who accustomed microsurgery. After acquiring 1 year experience microsurgery, using line production system intravascular stenting method, author along experienced supermicrosurgeons concludes that accurate channel possible safe rapid manner. tips described text video.

Abstract: Venous Anastomosis to Control Lymph Flow for Lymphatic Malformations

METHODS: With the same perforator anatomy of fibula flap5 and slight modification of harvest technique of fibula flap, the peroneal flap could be harvested within 1 to 2 hours. Between 1996 and 2015, 246 peroneal flaps and 114 ALT flaps were used in the head and neck reconstruction by the senior author (Dr. Yang). A variety of applications to different types of defects will be demonstrated to s...

Hemi-Intravascular Stenting for Supermicrosurgical Anastomosis

Background Although supermicrosurgical anastomosis is a widely known reconstructive microsurgical technique, it is difficult to perform. To expand the clinical use of supermicrosurgery, we used hemi-intravascular stenting (hemi-IVaS), which is performed by inserting an intravascular stent into one side of the vessel. We conducted lymphaticovenular anastomosis, free perforator flap transfer, and...

Side-to-End Lymphaticovenular Anastomosis through Temporary Lymphatic Expansion

OBJECTIVE The number of bypasses is the most important factor in lymphaticovenular anastomosis (LVA) for lymphedema treatment. Side-to-end (S-E) LVA, which can bypass bidirectional lymph flows via one anastomosis, is considered to be the most efficient bypass, but creation of lateral window to a small lymphatic vessel is technically demanding. To overcome the difficulty, we introduced S-E anast...

Abstract: Optimal Sites for Supermicrosurgical Lymphaticovenular Anastomosis: An Analysis of 1259 Anastomoses

RESULTS: LVA resulted in 1259 anastomoses using 949 lymphatic vessels at 794 surgical sites. The number of anastomoses per limb ranged from 1 to 16 (median 5). Multivariate analyses revealed that factors associated with larger lymphatic vessel (0.5 mm or larger) were age [65 or older; odds ratio (OR) 1.403], radiation history (OR 1.622), incision in thigh/leg (compared with groin; OR 1.607/1.62...

[Chyluria treated with inguinal lymphangiovenous and lymph node-venous anastomosis: a case report].

Stripping and ligation of the renal lymphatics has been a standard operation for patients with chyluria in Japan. However, recently, inguinal lymph node-saphenous vein anastomosis and lymphangiovenous anastomosis in the spermatic cord or the lower limb are introduced as a microsurgical treatment of chyluria with good result from China. We present a case of 50-year-old female with chyluria, whic...
